Our board-certified physicians direct complete primary care, specialty care and urgent care services for those who cannot leave home to get to doctor's visits. At Physician House Calls, our house call physicians, nurse practitioners, physician assistants and care team deliver compassionate, high-quality care to patients in their homes or places of residence. Our providers focus on each unique patient to deliver the appropriate care and emotional support to our patients and their families. Our office staff supports our providers and our patients in real time on a daily basis to help to improve overall patient satisfaction.

As a Physician, you will:

Be responsible for providing medical services and treatment to patients and families as home visits, principally in assisted and independent living facilities, serving as a clinical resource for staff and interdisciplinary team. Examine adult patients to ensure proper injury care and disease prevention, diagnosis, treatment, and recovery. Requests necessary tests, and follow-up home visits, and refers patients to specialists, as necessary.

Essential Functions:

Provides independent primary care, consultation and advice to others as a medical doctor. Makes patient home visits, principally in assisted and independent living facilities, assumes on-call duties, exercise prescriptive authority and directs Interdisciplinary Team Meetings.
Provides care for patients in a multitude of settings, including home visits, visits to assisted living and independent living facilities, hospitals and nursing homes.
Acts as the collaborating physician for any nurse practitioners or other advanced practice nurses employed by Kindred. Serves as a clinical resource to clinical staff. Participates in the education and mentoring of the nursing staff to promote quality of patient care.
Provides assistance in insuring Kindred is compliant with the "meaningful use" criteria in its electronic medical record implementation.
Provides night/weekend call coverage periodically.
Provides teaching, leadership and other administrative services within Kindred as reasonably requested.
Provides appropriate internal and/or external clinical education.
Attends team meetings, nursing meetings and staff meetings regularly.
Adheres to practices of confidentiality regarding patients, families, staff and Kindred.
Keeps and maintains appropriate records relating to all professional services rendered.
Must maintain appropriate licensing and continuing education requirements.
Required Skills
Requires a degree in medicine from an accredited medical school. BC/BE in Geriatrics, Internal Medicine, Palliative Care/Hospice or Family Practice.
Medical license to practice in state.
Current driver's license. Must have reliable transportation and insurance.
May require at least 2-4 years of internal medicine experience. Experience in Geriatrics, Internal Medicine, Palliative Care / Hospice or Family Medicine.
Effective interpersonal relations and communication skills.
Experience working in a collaborative care team environment and functioning autonomously with minimal supervision.
Demonstrated ability to manage clinical aspects of terminal and/or palliative care. Demonstrated ability to learn quickly, remain current on home health, hospice and/or palliative care trends and practice evidence-based care.
Ability to organize tasks, develop action plans, set priorities and function under stressful situations.
Knowledge of Medicare, Medicaid and other payers reimbursement and coding for all levels of service.
Ability to work flexible schedule and travel locally.
Ability to do heavy lifting, bending, pulling, pushing and standing. Prolonged standing and walking required. Requires working under some stressful conditions to meet deadlines and patient needs and to meet patient/family individualized psycho social needs. Requires hand-eye coordination and manual dexterity.
Meets mandatory continuing education requirements of Kindred and licensing board.
***This position will cover/support South Denver - in the Parker, Highlands Ranch, Lone Tree area.
