Ability Connection Colorado is looking for a full-time & part-time employee for this position.

The Client Manager will report to the Program Manager as apart of the Statewide Services department, and provide guardianship responsibilities to incapacitated clients.

Core Responsibilities:
-Conduct site visits and social assessment checklists regarding clientele throughout Colorado.
-Address and respond to clients concerns, including medical issues
-Maintain case files for each client and enter client data into database
-Oversight of client's personal financial needs account
-Maintain confidentiality with regard to all client information

Requirements:
-Associates degree in Human Services or related field
-1 year guardianship or client management or social work experience
-6 months experience working with vulnerable adults including mental health issues
-Must have valid CO driver's license and insurable MVR
-Requires some travel throughout Colorado
